Case Management assists children and adolescents and their families in the access, coordination, and monitoring of resources and services.  Services are provicec to assess on two level: resource coordination and intensive case management.

Resource coordination is available to clients with a mental health diagnosis and who are currently receiving or are in need of mental health services from two or more human service agencies or public systems, such as education, child welfare, and/or juvenile justice.

Intensive case management is available to clients with serious and persistent mental illness or serious emotional disturbance who meet eligibility criteria, including children who are returning from community inpatient units or other out-of-home placements and/or children who are recommended as needing mental health serservices by a local interagency team.
